1  Visit www.esb.mp.gov.in. and go to Citizen Services 
2  Read the notification, eligibility conditions, and instructions carefully before proceeding.
3  Click Continue to start the online application form and fill in all required personal, educational, and service details accurately.
4  Upload your colour photograph, signature, and handwriting declaration in the specified format, along with your caste/disability certificate and appointment order where applicable.
5  Pay the ₹2,000 application fee (plus applicable portal charge) via debit/credit card, net banking, or an authorised MP Online kiosk.
6  Review the completed form and submit it. Save/print the application form and payment receipt for your records.
7  Use the correction window (17 September–6 October 2026) if you need to fix any error, subject to the applicable correction fee.
