3. Fresh applicants should use Annexure-II.
4. Existing ESIC panel Advocates should use Annexure-III.
5. Fill out the prescribed application form in the required format.
6. Attach self-attested copies of all required documents.
7. Include details of your professional experience, cases handled, existing empanelments and other prescribed information.
8. Sign the required undertaking and declaration.
9. Place the application and supporting documents in an envelope.
10. Clearly superscribe the envelope with the prescribed "Application for the Empanelment of Advocates..." wording.
11. Send/submit the application to the ESIC Regional Office-Maharashtra so that it reaches the office by 28 August 2026, 05:00 PM15. For any other information, visit the official site.
