How to apply | 1. Read the advertisement carefully, Before you apply. 2. Eligible and interested candidates can appear for the walk-in interview scheduled on 15, 16, 17, 18 & 19 January 2024 at The Conference Hall, DRS Building, Near Pharmacy College CMOH Office, Jalpaiguri, West Bengal with the following documents. Application Form/Biodata. 3. For any other information, visit the official site. General Instructions: 1. No. of Posts may be changed in future. 2. Application Fees: For UR Rs.100/- only & For SC Rs.50/- only which is non refundable (Account Holder Name: District Health & Family Welfare Samiti, Jalpaiguri, Account No.: 442010100041511, IFSC Code: UTIB0000442 Bank Name: Axis Bank, Branch-Jalpaiguri). 3. The benefits of upper age concession up to 5 years are admissible only to SC & ST Candidates of West Bengal and up to 3 years to OBC-A & OBC-B Candidates of West Bengal only 4. In case OBC candidates category 'A' or "B" must be mentioned specifically in the caste certificate otherwise the candidate will be treated in 'Unreserved" category. 5. One candidate may apply for multiple posts if hel she is eligible. 6. Multiple applications for single post liable to cancelation of candidature. 7. The selection committee reserves the right to reject any online application which is fare not properly filled up. 8. Failure to submit any requisite documents liable to cancellation of candidature at the time of physical verification. 9. The engagement of above-mentioned posts are purely contractual basis. 10. The service may also be terminated by one (01) month's notice from either side. 11. All types of correspondences related will be done by online procedure only viz. Online notice board, exclusive candidate login portal. 12. No TAIDA will be paid to the candidates for the selection test 1 interview. 13. If the aggregate mark after final selection is equal then preference is to be given to the candidate inter se-senior in age as on 01.01.2024. 14. Any omission/suppression of information shall lead to rejection of application or candidature at any stage of the process without further intimation. The conditions so prescribed shall not be relaxed. 15. Candidates are instructed to not send (postal/ courier/ by hand) hard copy of application form & other relevant documents at this end. 16.
