1. Read the advertisement carefully, before you apply.
2. Click on “Recruitment/Result” → “Online Application forms/Admit Cards” or directly go to https://www.mphc.gov.in/exam-details.
3. Click on the Application Form link for “Suitability Test – 2025”.
4. Login using your credentials, verify pre-filled data, fill remaining fields, upload signature (JPEG format) and submit the form.
5. Take a printout of the finally submitted application form for future reference.
6. No correction is allowed after final submission.
