2. Eligible and interested candidates can appear for the walk-in interview scheduled on 04 August 2025 at The office of Medical Superintendent, ESIC Hospital, Silvassa Road, Chanod, Vapi, Valsad, Gujarat with the following documents.

iv. Age proof and other testimonials.

General Instructions:

1. Applicants having complete Bio-Data as per format uploaded (Annexure A-1) on ESIC website along with duly affixed recent passport size photograph, one set of attested photocopies of testimonials (including MBBS pass out marks sheets & certificates, 10th passing certificate as proof date of birth and SC/ST/OBC/PH Certificate) and other relevant documents including MCI / State registration certificate, may report to the office of Medical Superintendent, ESIC Hospital , Silvassa Road, Chanod, Vapi, Distt.- Valsad (Gujarat), on 04/08/ 2025 up to 11:00 AM and any application received thereafter will not be entertained.

2. Candidate seeking reservation benefits for SC/ST/OBC/EWS must ensure that they are entitled to such reservations per eligibility prescribed in the Notice. They should also be in possession of the required certificates in the prescribed format in support of their claim as stipulated in this Notice. OBC candidates should ensure that they are in possession of valid OBC Certificate issued within such issued by the authority mentioned in the format.

3. The candidate claiming reservation / age relaxation on grounds of belonging to OBC should submit the community Certificate in prescribe proforma vide Govt. Of India, Department of Personnel and Training OM No.36036/2/2013-Estt(Res). Date 30.05.2014 failing which the benefit of reservation / relaxation will not be given or their application shall be rejected and no request/ correspondence will be entertained. Certificates issued in other format will not be accepted and the candidates will be treated as “General Category” for all purpose. (format enclosed).

4. Those who belong to SC/ST/OBC category should produce valid caste certificate issued by the Tahsildar/Revenue Divisional Officer. OBC certificate must be as prescribed vide DoPT, Govt of India OM No. 36012/22/93-Estt.(SCT) dated 15.11.1993 which should certify that the candidate do not belong to anyone of the Creamy Layers. 5. Candidates claiming reservation under EWS category should produce their EWS certificate as prescribed vide DoPT, Govt of India OM No. 36039/1/2019-Estt.(Res) dated 31.01.2019. (format enclosed).

6. Candidate suffering from 40% or more of the relevant disability shall only be eligible for the reservation for PH category. Certificate of disability in original from any Govt. hospital should be submitted at the time of interview.

7. The selected candidate will have to furnish a DD for an amount of Rs. 30,000/-(Rupees Thirty Thousand Only) drawn in favour of ‘ESIC fund A/c No 1’ payable at Vapi, towards security deposit.

8. Candidates working in Govt. Service should produce NOC from their department at the time of interview. Other terms and conditions shall be as per Govt. of India rules adopted by ESIC.
