A Supreme Court-appointed expert committee has established minimum standards for Intensive Care Units (ICUs) in hospitals across India, creating a uniform benchmark for infrastructure, staffing, and patient care. The initiative aims to ensure that hospitals claiming to provide ICU facilities meet essential clinical and safety requirements, particularly for critically ill patients.

The Supreme Court endorsed the guidelines as “practical, implementable, and necessary,” noting that they are expected to enhance intensive care services across both urban and rural healthcare systems.

Mandatory ICU Infrastructure Standards Introduced

Under the new framework, every ICU must have a dedicated area with convenient access to emergency services, operation theatres, and laboratory facilities. Hospitals are required to maintain uninterrupted power backup, proper sanitation systems, and an internal layout designed to support emergency medical care.

Each ICU bed must include oxygen supply, suction facilities, and adequate electrical outlets. The list of essential equipment includes multiparameter monitors, ventilators, defibrillators, crash carts, infusion pumps, syringe pumps, glucometers, and ECG machines.

These measures are intended to standardise the minimum operational capacity of ICUs across hospitals nationwide.

Round-the-Clock Staffing Made Mandatory

The new norms place strong emphasis on staffing requirements. Every ICU must be supervised by trained doctors and staffed continuously throughout the day and night. The guidelines also mandate higher nurse-to-patient ratios compared to general wards.

The prescribed ratio ranges from 1:2 to 1:3 for standard ICU patients and up to 1:1 for critically ill or ventilated patients. In addition, allied healthcare professionals must be available to support ICU services.

Continuous patient monitoring, life-support systems, and strict infection control measures have also been made compulsory.

ICUs Categorised into Different Levels

The report, titled “Guidelines for the Organisation and Delivery of Intensive Care Services,” was prepared by a three-member committee and later reviewed by medical experts. It introduces a tier-based classification of ICUs, ensuring that even basic-level units comply with clearly defined standards.

The committee included AIIMS doctor Nitish Naik, Additional Solicitor General Aishwarya Bhati, and advocate Karan Bharioke. The panel emphasised that while standards are fixed, clinical judgment should continue to guide decisions regarding the level of care required for each patient.

States Directed to Submit Implementation Plans

The Supreme Court has directed all states and Union Territories to submit action plans by May 18, outlining existing gaps and strategies for implementing the new guidelines.

Experts highlighted that the framework acknowledges practical challenges in rural and remote regions. To address specialist shortages in smaller centres, interim staffing arrangements and support through tele-ICU and e-ICU systems have been recommended.

Overall, the new standards are expected to strengthen critical care infrastructure and reduce disparities in ICU quality across India’s healthcare system.
