A recent rise in shigellosis cases has been reported in Kerala, creating public health concerns due to the disease’s highly contagious nature. This infection, caused by Shigella bacteria, is a significant cause of diarrhoeal illnesses, particularly in areas where sanitation and hygiene conditions are inadequate.

What is Shigellosis?

Shigellosis is a bacterial infection caused by a group of microorganisms known as Shigella. It mainly targets the intestines and results in acute diarrhoea. The disease is among the most common bacterial causes of diarrhoeal illness worldwide. Humans serve as the only natural hosts of the bacteria, which makes person-to-person transmission a major factor in its spread.

Transmission and Risk Groups

Shigellosis spreads primarily through the faecal-oral route, usually when food or water becomes contaminated or through direct contact with an infected person. Poor hygiene practices greatly increase the chances of transmission. The infection can also spread through close personal contact, including sexual contact. Young children, especially those below five years of age, are at the highest risk. Other vulnerable groups include elderly individuals, malnourished people and those with weakened immune systems.

Symptoms and Clinical Features

The hallmark symptom of shigellosis is diarrhoea, which may contain blood or mucus and can last for several days. Additional symptoms include abdominal pain, fever, vomiting and a frequent urge to pass stool even when the intestines are empty. The severity of the disease varies depending on an individual’s overall health and immune strength.

In many cases, shigellosis is self-limiting, and mild infections may resolve without specific medical treatment. However, severe cases may require antibiotic therapy and proper medical attention. Preventing the disease relies heavily on maintaining good personal hygiene, practicing safe food handling, ensuring access to clean drinking water and improving sanitation systems, all of which are crucial for controlling outbreaks and reducing the overall disease burden.
